**Location: Bakersfield Heart Hospital**

**Position Summary**

Reporting to the Director of Quality responsible for the coordination of the disease specific care programs. Provides leadership and coordination for the chest pain center and heart failure program accreditations, sepsis program and mortality reduction initiative.

Accountable for leading interdisciplinary teams to achieve patient care quality and outcomes based on evidence-based guidelines. Provides monitoring and analysis of data to identify trends and facilitate ongoing performance improvement to improve care of the designated patient populations

**Qualifications & Requirements**

* Current California nursing licensure

* BSN Preferred

* 3-5 years experience in direct patient care of the ACS patient in either ED, Cath Lab, CCU or telemetry unit, Quality improvement background preferred

**Duties & Responsibilities**

**Oversees the data collection, analysis and performance improvement process for designated patient populations:**

* Demonstrates expert knowledge of database management and analytics overseeing cardiac in-house and contracted data abstraction and reporting

* Synthesizes and interprets multiple, sometimes conflicting, sources of data; makes judgment based on an immediate grasp of the whole picture supported by data

* Extrapolates insights, designs reports and assists with preparation of presentations from NCDR, GWTG, ACC, Mission Lifeline, mortality reviews and other databases

* Integrates knowledge of accrediting agencies and evidence based practice guidelines to develop reports and streamline quality auditing and data analysis

* Conducts data analysis for cardiac/AMI and heart failure database(s) and provides trend reports for Cardiovascular Care Council, Medical Quality, Senior Leadership Team and other committees as assigned

* Demonstrates quality improvement skills in the development of new processes and sustainability of

* Serves as liaison to Administration and represents the ACC accredited programs within the hospital and community committees /workgroups

* Assists with serving as BHH KC STEMI Receiving Center program manager and EMS-STEMI program liaison, collaborating with SRC medical director, managing data/QI activities, providing timely feedback to field providers and STEMI Referral Hospitals, participating as a voting member on the KC STEMI QI

**Works collaboratively with BHH Medical Directors, Senior Leadership Team, Nursing Department, other clinical providers, and the community to assure continuity of care and quality outcomes:**

* Works with designated physician reviewers to identify trends or events and/or individual providers needing peer review/mentoring for professional practice

* Facilitates and recommends protocols and guidelines for best practice treatment of patients across the continuum of care

* Assists in the development of strategic plans and performance improvement processes and growth of service lines

* Works collaboratively with Quality Department and clinical departments staff/leaders to address PI

**Develops and facilitates education for new and current employees on the mission and vision of the facility related to specific patient populations e.g. acute coronary syndromes, heart failure, sepsis**

* Designs and facilitates annual education offerings in collaboration with the Medical Directors for CPC and HF for the interdisciplinary care teams

* Educates nursing and ancillary staff on evidence-based practices in the treatment of specific patient populations

**Facilitates active involvement and complementary contributions of others in team meetings and discussions regarding patient care practices and population-specific programs; involves/recruits diverse resources when appropriate to optimize outcomes:**

* Chair/Co-chairs designated team meetings

* Maintains compassionate and caring relationships with peers / colleagues, communicates respectfully and contributes to a supportive and healthy work environment

**Promotes compliance with The American College of Cardiologists (ACC), Mission Lifeline, KC STEMI System of Care/STEMI Receiving Center designation and other federal, state, and local quality initiatives and guidelines:**

* Oversees and coordinates the ACC Chest Pain Center accreditation

* Oversees and coordinates the ACC Heart Failure program accreditations processes

* Represents BHH and actively participates in Kern County STEMI System of Care QI committee meetings with local hospitals and

* Facilitates dissemination and implementation of heart failure best practices with EMS and community
