Implements plan of care initiated by the Registered Nurse. Provides accurate and timely documentation consistent with the plan of care. Collects assessment data and performs observation to report to and collaborate with RN(s) and physician(s). Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the Registered Nurse, Clinical Services Manager and/or Director of Clinical Services. Uses equipment and supplies effectively and efficiently. Participates in personal and professional growth and development. Performs other duties as assigned by the Registered Nurse. Prepares clinical notes and ensures documentation is completed on a timely basis. Participates in on call duties as required by the agency.
