We are looking to hire a dynamic Licensed Practical Nurse (LPN) to join our phenomenal team of Clinicians!The LPN will supplement nursing care needs of the home health care and/or hospice patient according to the developed plan of care and under the direction of an RN Field Case Manager.RESPONSIBILITIESCarries out treatments, teaching, and observation and assessment activities as described in the Plan of Care and delegated by the case managerMaintains records and reports according to regulatory requirements and company policies and proceduresProvides daily report to Clinical Manager and Case Manager to review status of cases; participates in monthly clinical record reviewParticipates in personal and professional growth and developmentFulfills additional performance responsibilities as assigned and as necessaryREQUIREMENTS:Must be graduate of a school of practical nursing that meets requirements of state of licensure, and licensed in state(s) of operationMust possess sound knowledge of nursing principles, procedures, and standards of practice as determined by state and federal regulationsA minimum of one year of clinical experience requiredExperience in community or home health strongly preferredCOMPENSATIONIN OUR COMPANY YOU WILL FIND:Diverse pay system and great earning potentialHoliday, weekend and on call additional payMileage reimbursementAdvanced orientation and annual educational programsFriendly, family oriented and caring working environmentGreat benefits package which includes health, dental and vision care, PTO, company-paid life insurance and a 401K Plan.
