The Patient Concierge is a first responder from our agency who is passionate about the services we offer and works directly with patient/family/authorized representative, or any other customer to insure optimal access. Keeps patient centered services and caring at the forefront, exceeding customer expectations, and insuring best practice and flawless care in all aspects.

Position Requirements
Education and/or Experience: High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
Excellent verbal and written communication skills.
Knowledge of medical terminology.
Computer skills.
Knowledge of Empath Health/Suncoast Hospice programs.
Basic knowledge of reimbursement for services.
Working knowledge of community resources.
Ability to listen, assess needs and coordinate care.
Possess appropriate license and vehicle for travel needed to perform the job responsibilities.
