A DAY IN THE LIFE OF A HOME CARE DIRECTOR

Full understanding and execution of all policies and procedures adopted and approved by the governing body.
Proficiency as it relates to knowledge of the licensing's regulations and subject matter expert on all Personal Care and Nursing Care questions.
Under the direction of the Senior VP of Clinical Operation, the DCS leads the region's Personal Care nursing staff and designated initiatives/departments in delivering excellent patient care to all clients as authorized by the client's physician and health care standards/regulations.
Responsible for nursing recruitment decisions and ensuring that each nurse is actively licensed and abides by the standards for care and provides the highest levels of service possible.
Assists the SVPCO in creating and updating clinically based policies and actively seeks continual improvement in efficiency and accuracy through quality assurance activities.
Ensures clinical and regulatory compliance of the region and that nurses are trained on documentation expectations and being a system user.
Analyzes data trends and assists in refining current or implementing new clinical programs that create the best quality of life for our clients, while still maintaining the efficiency needed for growth.
Provides regular training and updates to nursing staff to keep them abreast of techniques, services or regulatory changes that may affect the care provided.
Oversees all clinical compliance activities and needs for the region and is directly involved in all patient care complaints stemming from clinical or regulatory concerns; works to de-escalate the situation while seeking resolution.
Provides proper discipline activities as warranted; works directly with HR on all serious personal issues and follows the progressive discipline model.
Makes initial contact with the patients and explains services offered
Completes start of care assessments and documentation to onboard new patients
Completes follow-up visits as needed
Designs an individualized plan of care for each patient
Instructs assigned care team members on the needs and care plan details
Reports progress of care to involved parties
Follows up and maintains relationships with clients, always advocating on their behalf
Establishes relationships with community partners and referral sources
Measures, analyzes, and reports on clinical metrics and key performance indicators.
Acts as the Company representative in external partnerships and relationships.
Acts on behalf of the clinical department in the event of an audit or regulatory review within assigned region.
Maintain licenses, revalidations, and delegated contracting of all locations and licenses overseen.
Performing necessary start of care assessments for region, including as back-up for the assigned team.
Maintain client census assigned, ensuring compliance as it relates to clinical oversight and frequency of visits.
Back-Up representative for the clinical team in assigned region in the event of vacancies or time off.
Coaching, mentoring, supporting, and extending disciplinary action as required for assigned team through set reviews as well as throughout employment.
Responsible for setting and achieving goals on a quarterly basis.
Other duties as assigned.

QUALIFICATIONS:

Must process an active, valid Registered Nurse license for the state of Maryland and DC (VA RN license preferred as well).
Minimum of 2 years in a leadership nursing role
Experience working with an EMR systems and being very comfortable with technology.
Actively in good standing with CPR certification and health requirements for region.
Ability to apply and analyze data as it pertains to quality and compliance metrics.
Comfortable working collaboratively in cross-functional teams.
Ability to communicate effectively, both verbally and in writing, with all levels of an organization.
Excellent presentation skills.
Able travel on a regularly basis
