Medications Support Counselor positions opened within our agency (www.socialmodel.com).

Essential Duties And Responsibilities

  • Conducts nursing assessments, health history, and assesses vital signs as part of the admissions process.
  • Provides oversight in the resident s self-administration of medications.
  • Ensures that all residents are weighed upon admission, including taking their vitals.
  • Responsible for connecting residents with health care provider(s) for completion of admission required physical examination and TB clearance.
  • Responsible for conducting pain assessments of residents upon admission.
  • Responsible for informing pertinent information, reported symptoms, and medication changes to supervisor and treatment team.
  • Complete consents and referrals as necessary to meet the physical, mental, and substance abuse treatment needs of residents.
  • Responsible for documenting medication changes, PRNs, and medication responses for each resident.
  • Verify resident s medical insurance information (Medi-Cal and other third-party payors) with pharmacy and Business Office.
  • Work collaboratively with treatment team to assist in the provision of case management care for medication and pharmacy related services.
  • Responsible for maintaining accurate external medication logs, including resident s daily medication logs and medication inventory logs.
  • Responsible for providing residents with education regarding medication, medication indications, and medication side effects.
  • Responsible for providing education on M.A.T. (Medications to Assist with Treatment) for substance use disorders.
  • Maintain clean and orderly medication room.
  • Responsible for duties of monthly medication log publishing.
  • Responsible for explaining the Treatment Policies and Procedures medication policies to treatment team members.
  • Responsible for conducting staff trainings of the medication room guidelines and signing off new staff members before they can assist in the residents in the medication room.
  • Responsible for ensuring the timely ordering of medications, the proper destruction of medications, and the documenting of these activities.

EDUCATION And/or EXPERIENCE

  • Licensed LVN or LPT with experience in residential treatment care.
  • Able to complete at least 20 hours of continuing education per year in areas related to substance abuse.
  • Knowledge of 12-Step programs and/or philosophy.
