Summary: Supports the Mission, Philosophies and Goals of the Agency Provides services and supports to individuals in a variety of settings, by completing the following duties. Education/Experience/Licenses: High school diploma or general education degree (GED) is preferred; to perform this job successfully, an individual should exhibit a high degree of patience, empathy and professionalism in working with individuals with intellectual and developmental disabilities. Must be able to communicate clearly, including: reading, writing and speaking. Must have a working knowledge of word processing programs, email programs and a willingness to learn other computer programs with training. Must be able to work flexible schedules including variable shifts. If qualified, must be able to drive large passenger vans in a variety of traffic conditions. If qualified, must have valid driver s license and proof of current liability insurance. Essential Duties and Responsibilities for both Day and Residential Shifts: Assists with supervision of persons receiving services in vocational/residential settings. Provides supports to Individuals receiving services as described in their Person Centered Plan (PCP), Risk Assessment (RA), Individual Justice Plan (IJP), Works Assessment (WA), Consumer Medical Emergency Information (CMEI), and Positive Behavior Support Plan (PBSP). Remains awake and alert at all times throughout the duration of assigned shift Assists in training individuals in areas of work skills/behaviors, and provides assistance and/or direct supports to individuals as needed or per PCP, RA, IJP, WA, CMEI, PBSP with all activities of daily living, including personal hygiene/ toileting. Accurately documents supports provided using agency-generated data collection forms such as support logs, behavioral logs, incident reports, etc. Completes all required documentation by the end of assigned shift. Completes and submits incident reports and trend tracking reports as applicable. Provides transportation as needed and as assigned. Completes housekeeping duties or provides housekeeping assistance to maintain a clean and safe environment at both day and residential sites, and any agency-owned building or vehicle Dispenses medication to individuals as required, and per training. Must complete trainings as required within specified timeframe. (See Employee Handbook Section 2.11) Complies with all agency policies and procedures including HIPAA/Confidentiality policies. Works at all COF Training Services, Inc. locations as assigned. Efficiently utilizes the company s resources, to include but not limited to time management, staffing hours, food, company owned vehicles, etc. Other duties may be assigned. While performing the duties of this Job, the employee is regularly required to use hands and fingers to handle, or feel; reach with hands and arms; talk or hear and taste or smell. The employee is frequently required to stand; walk; sit; climb or balance and stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move a minimum of 50 pounds; push wheelchairs and provide stability support to people while walking. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision and depth perception.
