Responsibilities:
- Conduct comprehensive patient assessments and develop personalized treatment plans with measurable goals and timelines.
- Prepare and submit detailed clinical and progress summaries to facilitate effective collaboration with the treating medical professionals.
- Share pertinent patient information to ensure accurate evaluation by the multidisciplinary team.
Qualifications:
- Associates, Bachelor's, Master's, or Doctoral degree from an accredited physical therapy program.
- Active state license in Physical Therapy / Physical Therapy Assistant.

Job Type: Full-time
Salary: $50,000.00 - $120,000.00 per year depending on experience.
