Provides in home evaluation, direction and application of skilled Nursing Services to complement the patient s overall Plan of Care. Case Management ensures the overall patient condition is cared for by providing appropriate education and resources to the patient/caregivers, allowing them to manager their conditions and remain at home. Job Requirements Education Required: Graduate of an accredited nursing program Experience Preferred: Minimum one (1) year experience as professional nurse within the last three (3) years. Skills Ability to provide reliable transportation as required by nature of position. Licensure/Certification/Registration Required: RN License active in the state of Missouri Required: BLS must be obtained within 90 days Required: Active Driver's License
