Job Summary:Implementation, coordination andsupervision of all aspects of a comprehensive Health, Wellness and ClinicalProgram which addresses the needs of all Assisted Living Residents inaccordance with Victory Housing, Inc. Policies and Procedures, COMAR 10.07.14and Board of Nursing Regulations and Standards.Oversight includes:Providenursing assessments, care planning, care oversight and quality assurance forall residents. Maintain all charts toinclude, progress notes, identification of nursing interventions, treatments and assessments. Implement a Wellness Program to include weekly scheduled resident Health and Wellness Check-ups, to assure a proactive and preventative clinical approach. Oversight and coordination of all third partyvendors to include but not limited to Podiatrists, Dentists, Lab, Pharmacy,Home Health, Hospice etc. Fulfil therole of Clinical Leader in the community asan essential manager, resident advocate, case manager, delegating andpracticing RN.Areas of Responsibilities:Ensure that the attainment of Victory Housing's MissionStatement takes precedence over all decisions and actionsAssume the responsibilities of the CaseManager/Delegating Nurse as defined in the COMAR 10.07.14 RegulationsResponsible for all aspects of Pharmacy and Medication Management. Responsible for the monthly change-over ofmedication cards; accuracy of monthly medication delivery from Order to MAR;training staff on new medications, parameters, orders and special conditions;as well as updating the Emergency Sheet (electronically and hard copy). Critical areas of responsibility areIn conjunction with the Assistant Director, responsibilities include interviewing, hiring,mentoring, training, evaluating and managing each Medication Technician. Each Medication Technician is working under the license of the Health/Wellness/Delegating RN. Oversight and management of medication administration must be in adherence to the Nurse Practice Act as well asFederal, State and local regulationsWriting in all orders from physicians into the POS/MAR according to VHI policyMaking daily/weekly checks of MAR (EMR/Hard Copy) to ensure proper administration and documentationMaintain overall quality of the resident care program in accordance with all local, stateand federal regulations and the highest standards of nursing practiceWork closely with all staff as RN on issues. Proactively intervene whenever a status change is assessedImplement a comprehensive Health and Wellness Program designed toproactively address physical, psychosocial and rehabilitative needs of the residents. Include quality assurance and outcome measureswhich monitor trends and anticipate potential problem areasSchedule weekly Check-ups with each resident to monitor all aspects of the resident's conditionand intervene as indicated. Review Medications, Fall Risk, Vital Signs, Weights, Nutritional Status, Skin Assessments and as well asoverall Clinical, Psychological and Social Status Changes. Record information on Wellness AssessmentForms, in Progress Notes (weekly) and Community Log (as needed)Maintain regular communication with resident, family(in absence of ED) and physician on individual health care issues. When a status change is identified, be theprimary liaison with healthcare professional including pharmacy, lab andHospice etcActively participate in monthly care planning meetings, Quality Assurance Meetings, FamilyClinical Reviews, and Hospice meetings etcActively participate in RSD. Report all resident statuschanges, follow-up actions and next steps. Educateas needed. Participate in Monthly Staff Meetings and provide In-service Training as requested by EDCoordinate (in conjunction with ED) dietary needs with FSD and environmental needs with the MaintenanceTechAssist the ED/AD with identifying and resolving employee concerns, complaints,grievances and issuesAssists the ED in completing 90-day and annual performance reviews. When requested by ED, will assist in counseling caregivers and medication technicianAvailable 24/7 to support clinical needs of the ResidentsDirect the community programs for infection control,quality assurance, outcomes monitoring and other oversight programsReview all resident charts on a regular basis, checking for content,clarity, completeness and appropriateness of care. Responsible for the quality of all resident charts, includingcompliance with applicable regulations and the highest standards of nursingpracticeInstruct caregiving staff on clinical and nursing issuesAssess each prospective Resident prior to move-in. In partnership with ED, recommend appropriateness of placementand level of careOn day of move-in or within 24 hours, meet with theresident to discuss: Wellness Check-ups, establish baseline, start clinical chart and review/establishcare needs and schedule in coordination with ED/AD/RSDComplete clinical components and follow-up of resident individual service plans with the Executive Director and the caregiving staff. Complete reassessments and service plans for residents as needed. Responsible for completing all assessments as mandatedvia COMAR RegulationsCoordinate services of outside agencies providinghealth care or other assistance to residents, including home health agencies,physical therapists, hospice, laboratories, durable medical equipment suppliersand other professional servicesReview and sign-off on Incident Reports with theExecutive Director and Assistant Director. Establishintervention and documentation of follow-upAttend classes, seminars and meetings. Maintain a high level of knowledge regarding current nursing theory, trends and regulatorychangesScheduled hours are from 10:00 - 6:30 or 9:00 - 5:30 based on community preference. The schedule must be set and adhered toconsistentlyIn the absence of Executive Director and Assistant Director, the Health/Wellness/ Delegating Nurse isresponsible for the day to day operations of the communityOther duties as assignedRequirements:Must be current on all vaccinations including COVID- 19 andreceive CDC recommended booster doses. Job Qualifications :Must be agraduate of an accredited school of nursing with three years experience innursing, preferably in geriatric services. Musthold a current Maryland license as a registered professional nurse. Baccalaureate degree in nursing or related health field with geriatric emphasis preferred. Must have successfully completed the 20Hour Delegating Nurse Course within 60 days of employ.
