We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our hospice care team. The ideal candidate will provide end-of-life care, ensuring patient comfort and supporting family members during this sensitive time.
Key Responsibilities:
Monitor patient health and administer medications as prescribed.
Provide basic patient care, including hygiene assistance, ambulation, and feeding.
Collaborate with RNs and doctors to implement comprehensive care plans.
Educate and support patients' families on care techniques and end-of-life preparation.
Document patient care services and comply with health and legal standards.
Qualifications:
Valid LPN license with experience in hospice or palliative care preferred.
Strong communication, empathy, and teamwork skills.
Ability to work in a high-emotion environment, providing emotional support to patients and families.
